Job summary

Homeside Rehab is seeking a compassionate Physical Therapist to join our home health team, delivering pediatric therapy in patients' homes with schedule flexibility and a supportive environment.

You will evaluate, treat, and document care, working collaboratively with a multidisciplinary team and families to achieve optimal functional outcomes in a home-based setting.

Qualifications

• Active Florida Physical Therapy License required.
• Basic computer proficiency for electronic documentation.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Valid Florida driver’s license and reliable transportation for home visits.
• Pediatric experience preferred.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals.
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes.
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements.
• Collaborate with the clinical team to ensure coordinated and effective treatment.
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans.

Job Summary:

Job Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals

• Location: Orange Park, FL / Jacksonville, FL 32224
• Salary: $65 – $80 per hour
• Schedule Flexibility: Choose your own hours; day and evening shifts available, Monday to Friday, with weekend availability as needed
