Job summary

Homeside Rehab is seeking a compassionate Physical Therapist or PTA to join our home health team in Bay Shore, NY. You will deliver individualized, evidence-based therapy to patients in their homes, providing schedule flexibility and a supportive team environment.

Responsibilities include patient evaluations, treatment planning, and documentation, with a focus on improving functional outcomes and independence.

Qualifications

• Active NY Physical Therapy license required.
• Experience with electronic documentation and EMR systems.
• Strong clinical assessment and communication skills.
• Ability to work independently in a home health setting.
• Valid NY driver's license and reliable transportation for home visits.

Job Summary

Homeside Rehab is seeking a compassionate and skilled Physical Therapist or PTA to join our home health team. In this role, you will deliver individualized, evidence-based therapy to patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals

• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes

• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements

• Collaborate with the clinical team to ensure coordinated and effective treatment

• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Qualifications

• Active NY Physical Therapy License (PTA) — required

• Basic computer proficiency for electronic documentation

• Strong clinical assessment and interpersonal communication skills

• Ability to work independently in a home health environment

• Valid NY driver's license and reliable transportation for home visits

• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings

Job Details

• Job Type: Full-time, Part-time, Per Diem

• Salary: $65 - $80 per hour

• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
