Qualifications

Responsibilities

• Participates in evaluation, planning, recording and implementation of patient care programs.
• Supervises PTAs, therapy aides and interns as applicable.
• Develops and adjusts treatment plans based on patient needs and goals.
• Consults with physicians and rehabilitation team to design coordinated plans.
• Completes evaluations, progress notes, discharge summaries and education.
• Maintains accurate documentation and communicates progress with patient and family.
