Qualifications

Responsibilities

• Assess and plan care and treatment as prescribed by physician.
• Evaluate patient status and PT needs using objective and subjective measures.
• Develop and adjust treatment plans to patient goals and family preferences.
• Consult with physicians and rehab team to coordinate care.
• Perform PT interventions and patient treatments.
• Fabricate or modify adaptive equipment and positioning aids as needed.
• Re-evaluate progress and modify treatment accordingly.
• Educate patients and families, including home exercise programs.
• Document evaluations, progress notes, discharge summaries, and communication.
• Manage schedule to meet department productivity standards.
• Attend staff and program meetings and in-services.
