Brooks Rehabilitation in Ocala, FL seeks a Physical Therapist to evaluate, plan, and implement patient care programs to restore function and maximize performance. You may supervise assistants and interns as part of a multidisciplinary team.

Responsibilities include reviewing history, developing individualized treatment plans, and coordinating with physicians and families to optimize outcomes. Strong documentation and patient-centered communication are essential.

Formación

• Active license to practice physical therapy in Florida.
• BLS certification required.
• Strong communication and teamwork; patient-focused care.
• Ability to document clearly and coordinate with the rehabilitation team.

Responsabilidades

• Assessment and Plan of Care and/or Treatment.
• Reviews patient history and interprets physician referral.
• Evaluates patient's status and PT problems and abilities using objective and subjective methods.
• Develops and adjusts treatment program based on patient/family needs and goals.
• Consults with physicians, other rehabilitation team members, patient and family to design a coordinated treatment plan.
• Performs patient treatment using appropriate physical therapy interventions.
• Fabricates and modifies adaptive equipment and positioning devices.
• Re-evaluates patient's status and environmental factors to modify/terminate the treatment program.
• Maintains clear communication with patients and others regarding progress, home programs, and issues related to therapy.
• Prepares patient for discharge with instructions and home programs; recommends additional treatment as needed.
• Manages schedule to meet departmental productivity expectations.
• Attends staff and program meetings and in-services; completes documentation per policy.
• Maintains accurate and professional written documentation.
