Qualifications

Licensed Physical Therapist (PT) in Illinois
Basic Life Support (BLS) certification
Completion of required continuing education for acute stroke care

Responsibilities

Assists patient transfers with proper body mechanics to ensure safety.
Provides compassionate, respectful care and education to patients and families.
Designs and adjusts a plan of care with goals, outcomes and discharge plan.
Educates patients and caregivers using effective teaching methods.
Evaluates clinical findings to determine PT diagnoses and outcomes.
Performs examinations with reliable methods based on history and screening.
Documents in accordance with CTSCP67 policy.
Screens patients to determine eligibility and need for therapy services.
Oversees PTA and rehab technicians; provides feedback.
Completes annual education for HIP and knee replacement program requirements when applicable.
