Qualifications

Responsibilities

• Demonstrates ability to perform patient transfers with safe body mechanics.
• Designs and adjusts plans of care based on patient response and evidence.
• Educates patients, families, and caregivers using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes.
• Performs examinations with reliable methods relevant to the chief complaint.
• Produces documentation in accordance with CTSCP67.
• Screens patients to determine the need for therapy services.
• Uses support personnel appropriately and provides feedback.
• Completes annual education for DNV Hip and Knee requirements.
• Evaluates and implements intervention plans that are functional for patients.
• Uses standardized outcomes measures to reflect functional performance.
• Completes documentation to regulatory standards.
• Collaborates with patient, family, and others throughout rehab.
• Supervises PTA and rehab technicians per regulatory guidelines.
• Communicates discharge plans and equipment needs with the medical team.

Education

Bachelor's Degree in Physical Therapy

Licensed Physical Therapist (PT) in Illinois

BLS Certification (AHA)

Continued education in acute stroke care
