Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients across all ages in our Springfield, IL area. The role focuses on delivering a patient-centered plan of care, educating families, and ensuring safe transfers and documentation in line with CTSCP67 guidelines.

The successful candidate will hold a Physical Therapy license in Illinois (or eligibility), maintain BLS, and engage in required continuing education related to acute stroke care.

Qualifications

• Bachelor's degree in Physical Therapy or related field.
• Licensed Physical Therapist (PT) in Illinois or eligibility to practice.
• Basic Life Support (BLS) certification.
• Commitment to continuing education on acute stroke care.

Responsibilities

• Evaluates and treats patients of all ages and adjusts plans of care as needed.
• Performs patient transfers with safe body mechanics.
• Designs plan of care integrating goals, treatment, outcomes, and discharge.
• Educates patients, families, and staff using effective teaching methods.
• Documents clinical findings and outcomes per policy.
• Supervises PTA and rehab technicians where applicable.
