Job summary

Carle Health is seeking a licensed Physical Therapist in Illinois to evaluate and treat patients of all ages and adjust plans of care accordingly. You will serve as a clinical resource for staff and students and collaborate with physicians to optimize outcomes.

You will perform exams and tests, design plans of care, educate patients and families, and document progress in the EMR. BLS within 30 days and IL PT license are required.

Qualifications

• BLS certification within 30 days (AHA).
• Licensed Physical Therapist (PT) in Illinois (IDFPR).
• Bachelor’s Degree in Physical Therapy is required.

Responsibilities

• Demonstrates ability to transfer patients with assistance from bed, chair or floor.
• Maintains safe body mechanics to protect staff and patients.
• Designs and adjusts a care plan with goals, treatments, outcomes and discharge plan.
• Educates patients, families, caregivers, and other health care professionals.
• Evaluates findings to determine PT diagnoses and outcomes; selects examination methods.
• Produces documentation per CTSCP67 and EMR standards.
• Screens patients for therapy eligibility and uses appropriate personnel as needed.
• Communicates progress and recommendations with referring providers.
