Job summary

Carle Health is seeking a licensed Physical Therapist in Illinois to evaluate and treat patients of all ages, developing and adjusting care plans based on patient response and evidence. You will educate families and collaborate with the care team to achieve positive outcomes.

The role requires active BLS within 30 days, ongoing continuing education for acute stroke care, and supervision of PT support staff.

Qualifications

Licensed Physical Therapist (PT) in Illinois.
Basic Life Support (BLS) within 30 days - American Heart Association (AHA).
Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.

Responsibilities

Demonstrates ability to perform patient transfers with assistance as needed, using safe body mechanics.
Treats patients with dignity and empathy, staying engaged to support care.
Designs and updates plans of care with goals, treatment, outcomes, and discharge planning.
Educates patients, family, caregivers, students, and other health care providers.
Evaluates findings to determine PT diagnoses and outcomes of care.
Performs examinations and selects appropriate methods based on history and screening results.
Documents in accordance with CTSCP67 and standards.
Screens patients to assess therapy needs and effectiveness of services.
Supervises PTA and rehabilitation staff per regulatory guidelines.
Communicates patient status, discharge recommendations, and equipment needs to medical team.
