Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor.
• Uses safe and appropriate body mechanics to reduce risk of injury to self, staff and patients.
• Designs and adjusts a plan of care that integrates goals, treatment, outcomes, and discharge plans.
• Educates patients, family, caregivers, students, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination and selects reliable examination methods relevant to chief complaints and history.
• Documents in the EMR according to policy.
• Communicates progress and recommendations to referring providers.
