Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Practices safe body mechanics to protect patients and staff from injury.
• Develops and implements plans of care with goals, outcomes, and discharge plan.
• Educates patients, families, caregivers, and staff using effective teaching methods.
• Performs examinations to determine PT diagnoses and outcomes.
• Documents evaluations and treatment in the EMR per policy.
