Job summary

PACS in Phoenix, AZ is seeking a Staff Physical Therapist to evaluate and treat residents, develop individualized treatment plans, and maintain documentation in medical records. The role involves communicating with families, physicians, and the health team, and training facility staff on an ongoing basis.

You will supervise PT assistants, coordinate discharge planning, participate in care conferences, and ensure infection control and safety procedures while maintaining confidentiality.

Qualifications

• Licensed Physical Therapist in the state; maintain CE requirements.
• Preferred advanced degree (Masters/Doctorate) in PT.
• Proficient with Net Health and PCC web-based tools.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Supervise PT assistants and aides in resident care.
• Communicate progress with physicians, staff, residents, and families.
• Maintain accurate and timely documentation in medical records.
• Participate in care conferences and quality improvement activities.
