Job summary

PACS is seeking a Staff Physical Therapist to evaluate and treat residents, and to communicate with families, physicians and the health team. The role includes maintaining documentation in medical records and conducting in-services for facility staff.

Responsibilities include developing treatment plans to improve mobility, strength, balance, and independence; supervising PT assistants; coordinating progress and discharge planning; and participating in care and rehab meetings.

Qualifications

• Bachelor’s degree in Physical Therapy; state licensure to practice in the state.
• Masters or Doctorate in Physical Therapy preferred.
• Proficient with Net Health and Point Click Care data entry systems.

Responsibilities

• Evaluate residents to determine PT needs, rehab potential, and functional goals.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise PT Assistants and aides in resident care and activities.
• Communicate progress and discharge planning with physicians, nursing staff, families.
• Participate in care conferences, utilization reviews, and rehab meetings.
• Complete timely, accurate documentation per requirements.
• Maintain infection control and safety protocols; educate residents and caregivers.
• Report equipment needs and contribute to quality improvement initiatives.
