JOURDANTON is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and coordinate care with physicians and the nursing team. You will supervise PT assistants and students while ensuring accurate documentation and adherence to facility policies.

The role requires a Bachelor's in Physical Therapy, state licensure, and ongoing continuing education. Proficiency with Net Health and Point Click Care is preferred; strong communication and teamwork are essential.

Qualifications

• Bachelor's degree in Physical Therapy required or equivalent.
• Licensed to practice Physical Therapy in the state; continuing education per state rules.
• Proficient with Net Health and Point Click Care.

Responsabilités

• Evaluate residents to determine PT needs and rehabilitation potential per physician orders and policies.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and trainees in resident care and activities.
• Communicate progress and discharge plans with physicians, nursing staff, and families.
• Complete timely documentation in compliance with facility and payer requirements.
• Maintain a safe treatment environment and educate residents and families on home programs.
