Qualifications

• Active Physical Therapist license in South Carolina.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans.
• Provide therapy to improve strength, balance, mobility and function.
• Monitor progress and adjust plans as needed.
• Maintain accurate clinical documentation.
• Communicate with physicians, residents, families and care team.
• Participate in care planning and discharge planning.
• Conduct staff education and in-service training.
• Supervise PTAs, aides and students.
