Job summary

Fleetwood Post Acute in Easley, SC is seeking a Physical Therapist (PT) to evaluate and treat residents to improve mobility and independence. You will collaborate with physicians, families, and interdisciplinary team members to deliver high-quality, patient-centered care.

Qualifications

• Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred).
• Active, unencumbered Physical Therapist license in South Carolina (required).
• Must maintain all continuing education and remain in good standing with the State Board.
• Experience in a skilled nursing or rehab setting preferred.
• Familiarity with PointClickCare (PCC) and Casamba preferred.
• Strong clinical, leadership, and communication skills.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans.
• Provide therapy to improve strength, balance, mobility, and overall function.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and compliant clinical documentation.
• Communicate with physicians, residents, families, and care team members.
• Participate in care planning, rehab meetings, and discharge planning.
• Conduct staff education and in-service training.
• Supervise PTAs, aides, and students.
