Job summary

Team Rehab is seeking patient-focused Physical Therapists for our Indiana clinics, including locations in West Lafayette. You will evaluate, diagnose, and treat patients to restore mobility, manage pain, and achieve measurable outcomes while supporting a team-oriented, evidence-based practice.

Qualifications

• Doctorate or Master’s degree in Physical Therapy.
• Valid Physical Therapy license (or eligibility to obtain).
• Commitment to diversity, equity, and inclusion.

Responsibilities

• Evaluate, diagnose, and treat patients to help them recover from injury, manage pain, and restore mobility.
• Provide excellent health care for patients, ensuring good outcomes and satisfaction.
• Create measurable, functional goals and adjust plans of care as patients progress.
• Educate patients on treatment options, home exercises, and strategies to prevent re-injury.
• Supervise and guide techs during patient care setup and cleanup.
• Use evidence-based pain-management techniques to improve function.
• Determine discharge timelines and provide patients with strategies for long-term success.
• Document evaluations, progress notes, and discharges accurately and on time.
• Achieve a reasonable level of productivity (approx. 60 treatments in a 40-hour work week).
• Participate in clinic research projects and protocol development.
• Stay current on best practices through in-house CEUs and continuing education.

Education

Doctorate or Master’s degree in Physical Therapy

Valid Physical Therapy license
