Job summary

Team Rehab is seeking a licensed Physical Therapist to join our outpatient clinics across the Midwest. You will evaluate and treat patients, develop plans of care, and work toward measurable outcomes in a collaborative, therapist-owned network.

Qualifications

• Doctorate or Master’s in Physical Therapy is required or eligible.
• Valid Physical Therapy license (or eligible to obtain).
• Commitment to diversity, equity, and inclusion.

Responsibilities

• Provide excellent health care for patients; ensure good outcomes and satisfaction.
• Evaluate, diagnose, and treat patients to restore mobility and manage pain.
• Create measurable, functional goals and adjust plans as patients progress.
• Educate patients on treatment options and home exercises.
• Supervise and guide Techs during patient care setup and cleanup.
• Document evaluations, progress notes, and discharges accurately.
• Participate in clinic research projects and protocol development.
• Stay current on best practices through in‑house CEUs.
