The position requires current PT licensure in the practicing state, BLS, and requires effective documentation and collaboration with a care team to meet department standards and patient needs.

Qualifications

• Licensed Physical Therapist in the state of current practice.
• BLS Basic Life Support (required within 30 days for some facilities).

Responsibilities

• Evaluates patients and develops individualized treatment plans with clear goals.
• Provides direct therapy daily following the plan of care and standards of practice.
• Maintains accurate records of treatment and patient progress in the EMR.
