Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility partner. The role supports planning, organizing, and directing PT services in compliance with federal and state standards and regulations.

The position involves providing PT services, documenting and billing per procedures, and collaborating with an interdisciplinary team to ensure high-quality resident care. Licensure, ongoing CE, and strong evaluation skills are essential.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure/certification.
• Commitment to professional development.

Responsabilités

• Provide physical therapy services per standards and department procedures.
• Document and bill therapy services in NetHealth EMR.
• Evaluate residents to develop and implement treatment plans.
• Develop short and long-term goals for resident care.
• Communicate findings and plans with residents, families, and the care team.
• Complete compliant documentation for CMS expectations.
