Job summary

Evolve-Therapy-Services in Virginia seeks a Physical Therapist for an in-house Skilled Nursing Facility. The role supports planning, directing therapy services to ensure high-quality resident care and CMS-compliant documentation.

You will evaluate residents, develop treatment plans and goals, and collaborate with the interdisciplinary team while maintaining licensure through ongoing education. Positions available include full-time, part-time, or PRN.

Qualifications

• Must have a BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services per established standards and procedures.
• Document and bill therapies in CMS-compliant NetHealth EMR system.
• Evaluate residents and develop short- and long-term goals.
• Conduct regular screenings to determine need for intervention/treatment.
• Collaborate with interdisciplinary team and communicate findings to families.
