Job summary

Qualifications

• Licensed Physical Therapist in good standing with the state.
• Bachelor's degree required; Master's or Doctorate (DPT) preferred.
• Strong communication and teamwork skills.
• Experience with electronic medical records (Casamba, PointClickCare).
• Ability to solve problems, think critically, and adapt in a fast-paced environment.

Responsibilities

• Evaluate patients within 24 hours of referral.
• Develop and implement personalized treatment plans with physicians.
• Provide hands-on therapy aligned with care plans.
• Monitor and document patient progress; update plans as needed.
• Educate patients, families, and caregivers for recovery and discharge.
• Assist with discharge planning and recommend equipment.
• Collaborate with nurses and healthcare team for coordinated care.
• Supervise PTAs, aides, and students.
• Participate in care conferences and staff training.
