Qualifications

Responsibilities

• Evaluate patients within 24 hours (or 1 business day) of referral.
• Develop and implement personalized treatment plans in collaboration with physicians.
• Provide hands‑on therapy in alignment with approved care plans.
• Monitor and document patient progress, updating plans as needed.
• Educate patients, families, and caregivers for successful recovery and discharge.
• Assist with discharge planning and recommend appropriate equipment (e.g., walkers, braces).
• Collaborate with nurses and healthcare team members to ensure coordinated care.
• Supervise Physical Therapist Assistants (PTAs), aides, and students.
• Participate in care conferences, team meetings, and ongoing staff training.
