Job summary

Redwood Grove Post Acute is seeking a dedicated Physical Therapist to join our compassionate team in delivering high-quality rehabilitation services. You will evaluate and treat patients, develop personalized plans, and contribute to a holistic patient care approach.

As a licensed PT in California, you will implement evidence-based interventions, collaborate with healthcare professionals, and educate patients and families to support successful recovery and smooth care transitions.

Qualifications

• Bachelor's or Master's degree in Physical Therapy (PT).
• Current California Physical Therapy licensure.
• Ability to work collaboratively with multidisciplinary teams.

Responsibilities

• Evaluate and treat patients with a broad range of physical disabilities and injuries with personalized plans.
• Perform initial and ongoing assessments to monitor progress and adjust plans as needed.
• Apply evidence-based PT interventions, including exercise programs, manual therapy, and education.
• Collaborate with other healthcare professionals to develop comprehensive patient care plans.
• Communicate effectively with patients, families, and caregivers to support seamless transitions.

Responsibilities:

• Evaluate and treat patients with a wide range of physical disabilities and injuries, developing personalized treatment plans to achieve optimal outcomes
• Conduct thorough initial assessments and ongoing assessments to monitor patient progress and adjust treatment plans as needed
• Implement evidence-based physical therapy interventions, including but not limited to exercise programs, manual therapy, and education
• Collaborate with other healthcare professionals to develop and implement comprehensive patient care plans
• Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and education

Requirements:

• Bachelor's or Master's degree in Physical Therapy (PT)
• Current Physical Therapy licensure in California
