Job summary

Edward-Elmhurst Health seeks a Physical Therapist II for outpatient rehab in Rolling Meadows, IL. The role offers 40 hours per week with day shifts and two evenings until 7:30pm, plus occasional Saturday mornings.

Responsibilities include 1:1 patient care, evaluating and treating patients, and documenting care per department guidelines. You will develop treatment plans to improve function and reduce pain, communicate with the care team, educate patients and families, and supervise

Qualifications

• Degree in Physical Therapy from an APTA-accredited program.
• Current Illinois licensure or eligibility.
• CPR certification through the American Heart Association.

Responsibilities

• Provide 1:1 care and assess, treat, and care for patients.
• Develop and adjust treatment plans to regain function and reduce pain.
• Prepare and maintain patient records and billing documentation.
• Collaborate with physicians, nursing and rehab staff for optimal patient care.
• Educate patients and families; involve caregivers in the plan of care.
• Supervise paraprofessional staff, students and volunteers as directed.
