Job summary

Endeavor Health in Arlington Heights, IL is seeking a Physical Therapist II for our Outpatient Physical Rehab Services. This full-time role offers 40 hours per week with shifts including 8:30am-5:30pm on Mon/Wed, 10am-7:30pm Tue/Thu, and 8am-12pm Fri.

You will provide 1:1 care, assess and treat patients, develop and follow treatment plans, document thoroughly, and collaborate with physicians and staff to ensure optimal outcomes.

Qualifications

• Requires a degree in Physical Therapy from an APTA-accredited school.
• Current Illinois licensure or eligibility.
• CPR certification through the American Heart Association.

Responsibilities

• Provide 1:1 patient care and implement treatment plans.
• Assess and treat patients across developmental stages and conditions.
• Prepare and maintain patient records, billing, and documentation per guidelines.
• Collaborate with physicians, nurses, and rehab staff to optimize care.
• Educate patients and families and involve them in care planning.
