Endeavor Health is seeking a Physical Therapist II for outpatient rehab in Rolling Meadows, IL. This full-time position requires days with two evenings until 7:30pm and occasional Saturday hours. No travel.

You will deliver one-on-one care and create individualized treatment plans to improve function and reduce pain. Responsibilities include assessment, documentation, collaboration with physicians and staff, and supervising students as needed.

Formación

• Requires a degree in Physical Therapy from an APTA-accredited program and Illinois licensure or eligibility.
• CPR certification through the American Heart Association is required.

Responsabilidades

• Provide 1:1 patient care with no double booking.
• Assess, treat, and care for patients in assigned area per developmental stage.
• Develop and implement treatment plans to restore function, strength, and reduce pain.
• Document patient records, billing, and departmental guidelines.

Physical Therapy degree (APTA accredited)

Illinois licensure or eligibility

CPR certification (AHA)
