Endeavor Health in Arlington Heights, IL is seeking a Physical Therapist II for outpatient rehab services. This full-time role offers 40 hours/week with varied shifts: 8:30am-5:30pm, 10am-7:30pm, and 8am-12pm on Fridays.

You will provide 1:1 care, conduct assessments, develop treatment plans, and document care with collaboration from physicians and nursing staff. Requirements include a PT degree from an APTA-accredited program, current IL licensure or eligibility, and CPR certification.

Qualifications

• The completion of a degree in Physical Therapy from an A.P.T.A. accredited school, or the equivalent.
• Current Illinois licensure or eligibility.
• CPR certification through the American Heart Association.

Responsibilities

• Provide 1:1 Care - No double booking.
• Assess, treat, and care for patients in the assigned area, implementing plans based on developmental stages.
• Evaluate functional abilities and plan modalities to regain function and reduce pain.
• Administer and instruct patients in treatment plans and adjust as needed.
• Prepare and complete patient records and billing per guidelines.
• Communicate with physicians, nursing, and staff for optimal patient care.
• Involve patient and family in the care plan and educate as needed.
• Supervise paraprofessional staff and volunteers as directed.
• Adhere to safety and infection control procedures.
• Participate in trainings, meetings, and departmental programs.
