Job summary

Endeavor Health is seeking a Physical Therapist II for our Outpatient Physical Rehab Services in Rolling Meadows, IL. This full-time position (40 hours/week) requires days plus two evening shifts until 7:30pm and occasional Saturday mornings 8am-12pm a few times per year.

You will provide 1:1 patient care, assess and treat based on developmental needs, document records, and collaborate with physicians and staff to optimize care and outcomes for diverse patient populations.

Qualifications

• The completion of a degree in Physical Therapy from an A.P.T.A. accredited school, or the equivalent.
• Current Illinois licensure or eligibility.
• CPR certification through the American Heart Association.

Responsibilities

• Provide 1:1 Care - No double booking
• Assess, treat, and care for patients in assigned area; develop and implement treatment plans
• Evaluate functional abilities and plan modalities to regain function and reduce pain
• Document patient records, billing and documentation per department guidelines
• Collaborate with physicians, nursing and staff for optimal patient care
• Involve patient and family in plan of care; provide education and home programs
