Job summary

Qualifications

• Bachelor’s degree in Physical Therapy and state licensure are required.
• BLS certification is required per policy guidelines.
• Ability to interpret medical records and physician reports to inform care.

Responsibilities

• Evaluates patients and establishes care plans based on holistic assessment.
• Selects and administers assessment techniques and modalities.
• Documents evaluations, treatments, and progress notes in the record.
• Coordinates patient care and discharge planning with the team.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
