Job summary

Qualifications

• Bachelor's degree in Physical Therapy required.
• Active state license in Physical Therapy required.
• BLS certification required.

Responsibilities

• Provides direct therapy services to patients on a 7 days a week schedule including holidays.
• Evaluates patients and develops comprehensive treatment plans with modalities, exercise programs, groups and education.
• Gathers data from medical records, physician reports, patients and families; documents evaluations and progress.
• Documents evaluations, tests, progress notes, daily treatment notes and discharge planning.
• Communicates evaluations and progress to physicians, referrals and clinical staff; coordinates schedule and discharge planning.
• Supervises PTAs and Rehab Technicians/Aides as required for licensure.
