Qualifications

• Bachelor's degree in Physical Therapy required.
• New York State Physical Therapy License or temporary permit required.
• Good communication and interpersonal skills.
• 0–1 years experience as a Physical Therapist.
• CPR certification required.

Responsibilities

• Initial Assessment and Evaluation of Physical Therapy needs for assigned patients.
• Review medical charts and clarify orders for unfamiliar diagnoses.
• Select assessment tools and determine optimal treatment plans.
• Complete all required documentation for patient care and reimbursement.
• Participate in team meetings, case conferences and committees.
• Develop and implement discharge plans with patient and care partners.
• Provide education to patients and supervise fieldwork students.
• Evaluate progress and modify treatment as needed.
• Refer patients to other clinical services as appropriate.
• Develop measurable short and long-term goals in care plans.
