Responsibilities

• Maintains professional appearance and professionalism in performance of duties.
• Initial assessment and evaluation of physical therapy needs for assigned patients; develop and adjust treatment plans.
• Complete all required documentation for patient care and reimbursement with proper terminology.
• Participates in team meetings, case conferences and committees as assigned.
• Develops and implements discharge planning in conjunction with patient and care partners.
• Assists in clinical education and supervision of fieldwork students.
• Evaluates patient progress and modifies treatment interventions as needed.
• Implements treatment plans and protocols according to professional and health system standards.
• Develops a care plan with measurable goals and time frames.
