Job summary

LiveWell Homecare Agency, LLC is seeking a qualified Physical Therapist to provide primary rehabilitative care in patients’ homes, supervising PTAs and Home Health Aides. You will coordinate personalized care plans with physicians, nurses, social workers, and caregivers to meet goals.

The role involves assessing patients, supervising staff, ensuring regulatory documentation, and contributing to continuous improvement. Georgia licensure, BLS, and a valid driver’s license are required.

Qualifications

• Graduate of an accredited physical therapy program.
• Valid Physical Therapist license and BLS certification.
• Georgia Driver’s License and private vehicle insurance.
• Minimum of 1 year full-time physical therapy experience; home health experience preferred.

Responsibilities

• Assess patient needs and perform routine and complex therapies based on physician orders.
• Coordinate services throughout the episode of care, adjusting care plans as needed.
• Supervise and review care provided by support staff, ensuring quality and compliance.
• Educate patients and caregivers to promote independence and self-care.
• Communicate patient status changes to physicians and participate in team conferences.
• Ensure documentation complies with regulatory standards, including CMS and Joint Commission guidelines.
• Participate in performance improvement initiatives and ongoing professional development.
• Meet productivity standards and perform additional duties as assigned.

They develop and coordinate personalized care plans, ensuring goals are met through collaboration with multidisciplinary teams including physicians, nurses, social workers, and caregivers.
