Job summary

Cherry Hill Free Clinic, Inc. seeks a Physical Therapist to assess, teach, and deliver home health therapy per physician-directed plans. You will collaborate with an interdisciplinary team and provide updates to optimize patient outcomes.

Responsibilities include mobility and strength assessments, developing plans of care, documenting progress, and guiding patients and caregivers on home exercises. OASIS duties may be involved where allowed by state license.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification, driver's license, and proof of insurance.
• Preferred: experience in home health therapy and OASIS documentation.
