BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a Part Time role performing home health visits in Calvert County. The position services adult and geriatric clients on a per-visit basis.

Requirements:

• Availability to see patients Monday-Friday, 8:30 AM - 5:00 PM.

As a home care Physical Therapist, you will be part of a multidisciplinary team providing rehabilitative care to help clients remain at home.

Responsibilities include:

• Making home visits within designated territories.
• Performing diagnostic tests such as mobility, strength, balance, and safety assessments.
• Implementing wound care interventions using evidence-based dressings.
• Developing and revising individualized care plans including manual therapy, gait training, and balance interventions.
• Assessing and updating care plans, collaborating with nursing staff.
• Educating clients and families on rehabilitative activities.
• Documenting interventions and evaluations accurately using electronic tablets.

Qualifications:

• Current Maryland Physical Therapist license.
• Graduation from an accredited Physical Therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Proficiency with computers; EMR experience preferred.
