About BAYADA Home Health Care

BAYADA is seeking an experienced Physical Therapist (PT) for a part-time role providing home health visits in Calvert County. This position involves visiting adult and geriatric clients on a per-visit basis. The candidate must be available weekdays between 8:30 am and 5:00 pm.

Role Overview

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities

1. Make home visits within designated territories.
2. Perform diagnostic tests, including mobility, strength, balance, and safety assessments.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and update individualized care plans, including manual therapy, gait training, and balance exercises.
5. Assess and revise care plans, collaborating with nursing staff as needed.
6. Educate clients and families on rehabilitative activities to promote health and independence.
7. Document all observations and interventions accurately using electronic medical records.

Qualifications

• Current Maryland Physical Therapist license.
• Graduation from an accredited program approved by the American Physical Therapy Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with EMR preferred.
