Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a part-time opportunity performing home health visits for our office located in Gaithersburg, MD. This office services adult and geriatric clients on a per-visit basis in territories throughout Montgomery County, MD.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility and range of motion, transfer status, stability, gait and ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound healing, utilizing evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the care plan, collaborating with nursing in the multidisciplinary team as appropriate.
• Educate clients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.

Qualifications for a PT - Physical Therapist include:

• Current MD Physical Therapist (PT) license.
• At least one year of recent clinical experience as a PT.
• Graduation from an American Physical Therapy Association (APTA)-accredited program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) is preferred.
