BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for Full-Time, Part-Time, or Per Diem opportunities. The role involves performing Home Health visits for our York office, located in York, PA.

This office services adult and geriatric clients on a per-visit basis in territories throughout Adams County.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team, providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.

Responsibilities include:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Assess and revise the care plan regularly, collaborating with nursing in the multidisciplinary care team as appropriate.
5. Educate and instruct clients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications:

• Current PA Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association-approved program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
