Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for part-time or per diem home health visits in our senior living office located in Allentown, PA. This office serves adult and geriatric clients in senior living facilities throughout Lehigh County.

As a home care Physical Therapist, you will be a key member of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

• Making home visits to clients within designated areas.
• Performing diagnostic tests, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Implementing wound prevention and repair interventions using evidence-based dressings and topical agents.
• Developing and updating individualized care plans with manual therapy, gait training, balance, and other interventions.
• Assessing and revising care plans, collaborating with nursing staff as needed.
• Educating clients and families on rehabilitative activities to promote health, safety, and independence.
• Documenting observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications:

• Valid Physical Therapist (PT) license.
• Graduation from an APTA-approved program or accredited by the Committee on Allied Health Education and Accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency in computer skills; experience with EMR preferred.
