UVM Health - Home Health & Hospice is seeking a Physical Therapist for Adult Home Care on a per diem basis. The role covers serving Chittenden & Grand Isle Counties, VT, with weekdays availability and no weekends at this time.

You will evaluate, plan, and deliver therapy to help patients regain mobility and independence in their own homes. Join a supportive team with mentoring and ongoing education, enjoy flexible scheduling, and contribute to high-quality home health care.

Qualifikationen

• Bachelor’s degree or higher in Physical Therapy.
• 2+ years of general PT experience, or 1 year at PT I level with strong performance.
• Current Vermont Physical Therapist license or willingness to obtain one.

About The Role

As an Adult Home Care Physical Therapist, you will:

• Perform comprehensive evaluations and assessments
• Develop individualized treatment plans tailored to each patient’s needs
• Deliver skilled therapy interventions to improve mobility, strength, and function
• Support patients in remaining in their homes safely and independently

Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties. Per diem support all of the four territories.

Minimum Qualifications

• Bachelor’s degree or higher in Physical Therapy
• 2+ years of general PT experience, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
• Home health experience preferred, but not required, we can train the right person!
• Current Vermont Physical Therapist license, or willingness to obtain one
