Job summary

Visiting Rehab and Nursing Services (VRNS) is seeking a compassionate Physical Therapist (PT) to join our home healthcare team in Stoneham, Massachusetts. This per diem role offers flexible scheduling, mileage reimbursement, and competitive compensation up to $160 per visit, with a focus on delivering high-quality one-on-one care.

You will perform home-based PT evaluations, develop individualized treatment plans, guide therapeutic exercises, assess home safety, educate patients and caregivers,

Qualifications

• Current Massachusetts Physical Therapist license.
• Minimum of one year of clinical experience.
• Afternoon availability.
• Weekend availability (nice to have).

Responsibilities

• Providing comprehensive physical therapy evaluations.
• Developing individualized treatment plans.
• Providing therapeutic exercises and functional mobility training.
• Assessing patients' home environments and recommending safety modifications.
• Educating patients and caregivers.
• Monitoring patient progress and modifying treatment plans as needed.
• Collaborating with physicians and the care team.
• Completing accurate and timely electronic documentation.

Your Role as a Home Care Physical Therapist (PT):

As a Physical Therapist (PT) with VRNS, you’ll provide skilled physical therapy services directly in patients’ homes. You’ll work closely with patients and their care teams to promote mobility, independence, safety, and quality of life.
