The role emphasizes evaluations, individualized plans, and interventions to improve mobility, independence, and overall well-being for patients in their own homes, working with an interdisciplinary team.

Qualifications

• Bachelor's degree or higher in Physical Therapy.
• 2+ years general PT experience; home health experience preferred but not required.
• Current Vermont Physical Therapist license or willingness to obtain.

Responsibilities

• Perform comprehensive evaluations and assessments.
• Develop individualized treatment plans tailored to each patient.
• Deliver skilled therapy interventions to improve mobility, strength, and function.
• Support patients in remaining in their homes safely and independently.
