Job summary

UVM Health Adult Home Care is seeking a dedicated Physical Therapist to provide home-based therapy in the Chittenden & Grand Isle Counties of Vermont on a per diem basis. The role emphasizes helping patients maintain independence, improve mobility, and enhance overall well-being in their own homes.

You will join a collaborative interdisciplinary team of RNs, LPNs, LNAs, OTs, SLPs, Social Workers, and administrative staff to deliver patient-centered care.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• 2+ years of general PT experience, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance.
• Current Vermont Physical Therapist license, or willingness to obtain one.
• Home health experience preferred, but not required; training available for the right candidate.

Responsibilities

• Perform comprehensive evaluations and assessments.
• Develop individualized treatment plans tailored to each patient’s needs.
• Deliver skilled therapy interventions to improve mobility, strength, and function.
• Support patients in remaining in their homes safely and independently.
