Job summary

Crossroads Home Health and Hospice is seeking a dedicated Physical Therapist to join our home health team. The PT will assess patients in their homes, design individualized treatment plans, and provide hands-on therapy to improve strength and mobility.

Ideal candidates hold an active Physical Therapist license, have at least one year of home health experience, and can educate patients and families while coordinating with physicians and staff to achieve optimal outcomes.
