Job summary

Palms-Home-Care is expanding its home health team and seeking a skilled Physical Therapist to provide in-home care. You will perform evaluations, design customized treatment plans, and guide patients toward improved mobility and independence after illness or surgery.

The role requires current PT licensure, Florida-appropriate credentials, and strong communication. Experience with OASIS and Kinnser EMR is a plus; valid driver’s license and reliable transportation are required.

Qualifications

• Current Physical Therapist license
• Graduate of accredited PT program
• 1-2 years of experience preferred in Home Health
• Strong communication skills
• Ability to work independently
• Possesses and maintains current CPR certification
• Valid driver's license and reliable transportation
• Knowledge of OASIS
• Experience with EMR systems such as Kinnser
