Job summary

MaineHealth Home Care is seeking a Physical Therapist for Home Health and Hospice services. You will evaluate patients, develop plans, and deliver therapies to restore function at home.

You will follow APTA standards, collaborate with care teams, and help patients reach their maximum potential. A valid state license and BLS are required, plus a driver's license and reliable vehicle.

Qualifications

• Graduate of an APTA accredited Physical Therapist program.
• License as a Physical Therapist and BLS certification required or obtained within 30 days of start date.
• One year of experience in PT in hospital, long-term care, home health, school system or outpatient setting preferred.

Responsibilities

• Perform patient evaluations and develop plans of care.
• Deliver therapies to restore function, reduce pain and prevent disability.
• Comply with APTA standards and coordinate with the care team.

Summary:

The Home Care Physical Therapist performs evaluations, develops plans, and carries out treatments to restore function, relieve pain, and prevent disability following disease, injury, or loss of a body part; helps patients reach their maximum performance in accordance with the Plan of Care prescribed by the provider. Provides all aspects of care based on standards established by the American Physical Therapy Association. Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: Graduate of an APTA accredited Physical Therapist program required.
2. License/Certifications: Current applicable state(s) temporary/permanent license as a Physical Therapist required or in compliance with the state practice act. Current BLS certification required or must obtain within 30 days of start date. Positions in Home Care will require current valid Driver's License with vehicle available for work covered by liability insurance as specified by agency policy.
3. Experience: One year of experience working in a hospital, long-term care, home, school system, or outpatient Physical Therapy department preferred.
