Jobtailor is seeking a Licensed Physical Therapist to provide home-based therapy across Oak Brook area and surrounding locations. You will assess, plan, and implement physical therapy services for patients in homes and assisted living facilities, while coordinating with the care team and ensuring regulatory compliance.

Qualifications

Responsibilities

  • Provide physical therapy care in patient homes and assisted living facilities.

Job description

  • Provide physical therapy care in patient homes and assisted living facilities
  • Use appropriate tests, measures, and interventions to achieve patient outcomes
  • Document thoroughly to support reimbursement and regulatory requirements
  • Participate in quality and process improvement initiatives
  • Promote patient safety and reduce risk
  • Participate in staff meetings, agency in-services, educational activities, and interdisciplinary case conferences
  • Maintain current practice knowledge and develop clinical skills
  • Communicate, provide feedback, and resolve conflicts with healthcare team members
  • Make sound clinical decisions for patients, including those with complex problems
  • Evaluate clinical changes and implement updates to plans of care
  • Prioritize and organize patient care and provide clear handoff communication using SBAR and 5P's
  • Initiate referrals and discharge planning and coordinate interdisciplinary care
  • Build restorative relationships with patients and families
  • Educate patients and families on self-care, disease processes, medications, procedures, and infection prevention
  • Develop individualized plans of care addressing physical, psychosocial, comfort, and wellness needs
  • Assess readiness to learn and adapt patient education materials and approaches

Requirements
  • Licensed in the State of Illinois or eligible for licensure
  • Able to demonstrate compliance with state continuing education requirements
  • 1 year of clinical experience or strong spectrum of clinical affiliation experience
  • Basic knowledge of computer use, including Microsoft Word and email
  • Ability to learn computer-based scheduling and documentation systems
  • Professional verbal and written communication skills

  • Proficient use of home-based therapy equipment and assistive devices
  • Must provide or arrange own transportation to and from patients' homes and branch offices
  • Ability to work rotating weekends and holidays

  • Ability to lift up to 35 pounds without assistance; patient-handling equipment expected for heavier or non-assisting patients
  • Ability to perform bending, sitting, standing, stair climbing, pulling, pushing, reaching, carrying, lifting, ambulation assistance, patient transfers, and treatment on standard beds
  • Extensive travel within geographical boundaries in all weather and on all types of roads
  • Physical Therapist license in the State of Illinois
