Job summary

Jobtailor is seeking a licensed Physical Therapist to provide in-home care in Mishawaka, Indiana. You will evaluate patients, develop personalized treatment plans, and deliver PT services at patients' homes.

You will monitor progress, collaborate with a multidisciplinary team, document care, and supervise therapy staff as appropriate. A valid PT license and reliable transportation are required.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes and work locations.

Requirements

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
